This guide will explain the process of verifying your insurance before surgery. The estimated fees are based on information obtained from your insurance company. Please keep in mind that these fees will be subject to your deductible, co-insurance, and out-of-pocket costs.

Physician Fees

· Once the provider creates an order for surgery, we will contact your insurance company for eligibility and benefits for outpatient surgery within 3 to 5 business days from your original appointment.

· The insurance representative provides our office with your in-network and out-of-network benefits for outpatient surgery, as well as the current amount paid toward your deductible and out-of-pocket expenses.

· After obtaining the benefit information our office will provide you with estimated fees for the physician and facility. **Patient is responsible for obtaining anesthesia fee.

· Please note that your insurer may not pay all charges. If you have questions about your coverage, we recommend you contact your insurance provider.

· If your insurance company requires authorization for a specific procedure, we will submit an authorization request, which may take up to 15 business days.

· All physician fees for Arizona Hearing Center - Mark J. Syms, M.D. and outstanding balances must be paid through our website azhear.com under Patient Resources no later than 10 days prior to surgery date.

Select "Make a Payment," choose your birthdate, and enter the amount, either over the phone or in person.

​· Please note that any cancellation with less than 7 business days' notice from the surgery date is subject to a $250.00 cancellation fee. The cancellation fee is not covered by insurance.
