New Patient
- Complete all sections under My Health Tab
- Add all current medications - Our Team will not enter medications or health history on your behalf
- Add your Pharmacy
- Add your Primary Care Provider
- Add additional Doctors in your care team (Cardiologist, Pulmonologist, Neurologist)

Day Of Your Visit
- Arrive 20 minutes early
- Quiet zone: Please silence your cell phone
- No food or drinks
- We limit to 1 guest per Patient
- Kiosk check-in:
- Photo id and insurance cards required
- Co-payments, Deductible amounts collected
- Paperless office - payment receipts (if applicable) will be emailed
- Hearing test performed same day - we will not use outside testing
- If translation is needed, please bring someone with you
