To ensure a smooth surgery scheduling process, please complete these items as soon as possible:
- Call Banner Peoria Surgery Center 623-234-4532 for Medical clearance/Preassessment .
- Call the Financial Department 480-292-8541 ext 2 for a facility quote for your surgery.
- For Anesthesia questions please call a week prior to scheduled surgery as the provider varies.
- Login to the Patient portal azhear.ema.md to add/update your current Pharmacy under Surescripts Pharmacy.
