How to Upgrade Your Processor

Arizona Hearing Center provides clinical services and documentation for your upgrade. We do not provide any parts/device to patients.  The implant manufacturer provides the device and/or parts directly to patients and submits the claim to your insurance. The manufacturer’s customer service department will be your contact regarding insurance authorization of your upgrade.  

 

Once authorization is obtained, customer service may collect any applicable co-insurance payment from you. Once all requirements have been met, your device/parts will be shipped to your home.  If you have ANY questions regarding this process, please contact your implant manufacturer or your insurance company. Arizona Hearing Center has no ability to expedite or check the status of this process.

Steps to Upgrade Device

  1. Complete a medical evaluation at Arizona Hearing Center
  2. Pay processing fee for the Letter of Medical Necessity
  3. Contact the device manufacturer and place an order.
  4. After you receive the device, schedule an appointment for activation

Please ensure the kit is shipped to you. Our office will not accept your order.
